United States: A dermatophyte identified as Trichophyton mentagrophytes genotype VII (TMVII) has emerged as the culprit behind a nascent transmitted fungal infection, documented in four adults in the United States, as outlined in a recent issue of the Centers for Disease Control and Prevention’s Morbidity and Mortality Weekly Report (MMWR).

TMVII, a transmissible fungal pathogen, is responsible for genital tinea and often poses as dermatological conditions such as eczema or psoriasis, mentioned Dr Jason E. Zucker, an infectious disease authority from Columbia University Irving Medical Center, alongside colleagues, according to medscape.com.

“Dermatophyte contagions like TMVII propagate predominantly via direct dermal contact,” remarked Dr. Avrom S. Caplan, a dermatology expert from NYU Grossman School of Medicine, in an interview.

“In the United States, we are only aware of occurrences among MSM [men who have sex with men]. However, cases have surfaced in Europe involving individuals outside the MSM demographic, including travelers engaged in sexual tourism in Southeast Asia or their infected partners,” Caplan noted.

Between April and July 2024, four individuals were diagnosed with tinea, with fungal cultures and DNA sequencing pinpointing TMVII as the pathogen. These men, aged 30-39 and residing in New York City, reported recent sexual encounters with other men; one engaged in sex work, two had sexual relations with one another, and one had recently traveled to Europe, as mentioned in medscape.com.

The patients presented with cutaneous eruptions localized to the face, buttocks, or genitalia. Antifungal therapy resolved symptoms in all cases.

Prompt Clinical Attention Advised 

Healthcare providers should maintain a high index of suspicion for TMVII in sexually active individuals with genital lesions, especially if concurrent pruritic, scaly, or inflamed eruptions appear elsewhere on the body, Caplan emphasized.

“Moreover, if a patient presents with ringworm on areas like the buttocks or face, particularly with a history of sexual activity, clinicians should inquire about genital lesions,” Caplan advised. He also stressed the importance of thorough evaluation for other sexually transmitted infections (STIs) in patients diagnosed with TMVII, particularly within the MSM population.

Call for Enhanced Surveillance 

Continued vigilance is imperative to comprehend the epidemiology of TMVII and emerging mycoses, Caplan stressed. Clinicians are encouraged to consult dermatophyte registries through platforms like the American Academy of Dermatology for updated information.

“We urgently require improved diagnostic access and more rapid confirmatory testing to detect emerging dermatophyte strains and monitor antifungal resistance,” Caplan added. Current data suggest no evidence of antifungal resistance in TMVII, distinguishing it from T. indotineae, he noted, according to medscape.com

Challenges in Diagnosis and Reporting 

“Emerging infections frequently mimic non-infectious ailments, complicating diagnoses,” commented Dr. Shirin A. Mazumder, an infectious disease scholar at The University of Tennessee Health Science Center. She underscored the significance of educating clinicians through case studies and encouraging the reporting of atypical presentations to public health authorities.

Skin conditions unresponsive to or exacerbated by corticosteroid use may serve as diagnostic indicators of TMVII, Mazumder explained. The fungus’s clinical manifestations, often mimicking eczema or psoriasis, necessitate thorough sexual history assessments, as transmission stems from intimate contact.

Future Research Directions 

Optimizing treatment protocols for TMVII remains an open question, Mazumder observed. The efficacy of combination regimens involving oral and topical antifungals versus monotherapy demands further exploration. Other knowledge gaps include determining transmissibility rates, periods of infectivity, reinfection risks, and the influence of host immunosuppression on disease trajectory, as per medscape.com.

Investigating long-term sequelae, bacterial coinfections, and the impact on broader patient populations will also yield critical insights, Mazumder concluded.

“I want to thank my husband, President Trump, for standing with us on this effort,” Melania Trump stated, with a riot of pink roses in bloom behind her. 

What is the Take It Down Act? 

The bill, known as the Take It Down Act, commands social media platforms to promptly take down sexually explicit pictures and videos of others, including “deepfakes,” if they were uploaded without consent. It was created to support teen survivors. 

Melania Trump’s Mission to Protect Teens 

“Artificial Intelligence and social media are the digital candy for the next generation — sweet, addictive and engineered to have an impact on the cognitive development of our children,” the first lady stated, the New York Times reported. 

“But unlike sugar, these new technologies can be weaponized, shape beliefs and, sadly, affect emotions and even be deadly,” she added. 

This guide focuses on teenage victims of FGM. Mrs. Trump was determined to get the bill passed from the day of her husband’s second inauguration, the day Senator Amy Klobuchar, Democrat of Minnesota, first mentioned it over lunch. 

Like Be Best, Mrs. Trump’s anti-cyberbullying effort from the previous four years, the bill was a great fit for the first couple’s focus on battling digital meanness. 

Early in March, she visited Washington to lead a roundtable with girls who had fallen victim to online attacks and the father of a young boy who left the world because of internet “sextortion.” 

The event was emotional at times, and after it was over, Mrs. Trump took some of the guests to see the president address Congress the next day, where he discussed the bill, the New York Times reported.

Deadly Blast Rocks Palm Springs Clinic 

The blast occurred outside the American Reproductive Centers, a local fertility clinic, claiming one life and leaving four others injured. 

The explosion, which originated from a vehicle parked near the facility, tore through the area with devastating force. 

Emergency services quickly responded to the scene. Fortunately, no patients or clinic staff inside the building were harmed, although structural damage was reported. 

FBI Labels It Terrorism

United States: Hadi Matar, who attacked novelist Salman Rushdie last year, has been sentenced to 25 years in prison — the maximum for attempted murder. 

Matar received a conviction for second-degree attempted murder for the attack on Rushdie in 2022 while they were both at the Chautauqua Institution in New York. 

A man armed with a knife attacked Salman Rushdie as he was about to deliver a speech, inflicting injuries on the writer all over his body. 

Rushdie suffered severe damage to the eyes and nerves and is now partially blind because of the attack, npr.org reported. 

Rushdie wasn’t in the courtroom for Friday’s sentencing but wrote that he’s spent over 800 days in hospitals since the assault. 

Judge David Foley additionally ordered Matar to serve 7 years in prison on the charge of causing serious bodily injury to the event moderator. 

Rushdie’s novel The Satanic Verses created widespread outrage among many Muslims for its depiction of the Prophet Muhammad. 

Just before his passing in 1989, the Ayatollah Khomeini, the Supreme Leader of Iran, declared a legal judgment condemning Rushdie to death. 

Prosecutors from the Western District of New York argued that Matar was attempting to carry out the threat contained in the fatwa. 

Matar has affirmed that the fatwa didn’t inspire the attack but has explained to the New York Post that Rushdie had personally offended him by insulting Islam, npr.org reported. 

Matar told reporters that he had skimmed over just a few pages of The Satanic Verses.
